2. Adhere to the current Official Course Outline as approved by the Illinois Community College Board. 

3. Review the student’s eligibility for participating in an academic experiential learning assignment (GPA, prerequisites, etc.) 

4. Determine and discuss how the proposed intern program is relevant to the student’s educational objectives, how   

           to apply academic theory to the world of work, and is not work that a regular employee would perform. 

5. Determine academic assignments, monitor student progress and provide final evaluation/grade. 

6. Maintain contact and documentation in a management system with all student assessments for a period of three years.  

7. Any expectations for protecting confidential or proprietary information should be defined in writing at the beginning of the  

 

           Student Code of Conduct. You will also be representing the company you intern with, remain professional and adhere to company’s 

    code of ethics and safety policies.          

2. Comply with the location’s expectations of confidential or proprietary information.   

3. Fulfill tasks/responsibilities assigned by your location and achieve learning goals approved by your instructor of record. 

4. Notify your Instructor of Record in a timely manner of any changes to the Intern Program or if any problems develop during  

           the program, including reporting of incident/injury. 

5. Keep in contact with your Instructor of Record, arrange to debrief with your Instructor of Record; return this form to the instructor of  

2. Comply with federal laws prohibiting discrimination on the basis of race, color, national origin, sex, disability and/or military  

           status. 

3. If paid, accept the responsibility for providing Workers’ Compensation and liability insurance in accordance with the state law, and  

           agree to provide said coverage. 

4. Verify the position and duties by providing a job description with a defined beginning and end.  The dates shall correspond to the  

            academic/semester calendar. 

5. Provide an orientation, any precautionary safety instructions, training, and ongoing supervision for assigned duties. 

6. Provide regular feedback to the student regarding performance.
